I am a new Hazlewood student at Cedar Valley College that is a former Texas member of the U.S. Armed Forces.

I am an eligible dependent of a Hazlewood (Texas) veteran using benefits for the first time.

  • Provide Veteran's DD214 - Proving their military service and nature of discharge
  • Need Certificate of Eligibility (COE) or Benefits Letter from Dept Veteran Affairs - Provide proof of eligibility or ineligibility for GI/Montgomery benefits (Chapter 33/Post 911)
    (i.e DD1300 - Veteran died or is rated totally disabled for purposes of employability)
  • Child - Need Birth Certificate or Parent's/ Veteran's 2010 tax return. - Prove your dependency to the veteran
  • Spouse - Need legal marriage certificate or 2010 Tax Return - Prove your marriage to the veteran
  • Print out from the HH Loans website at www.HHLoans.com- regarding loan status
  • Print out from the https://www1.thecb.state.tx.us/apps/hsh/students
    Students must login online, printout, and provide their hours used to the school along with this application.
  • HE-V Packet completed and signed by the veteran
  • HE-D Packet completed and signed
  • Tuition Account Summary for the term
